What Is TB-500?

TB-500 is the informal name for a synthetic version of a seven-amino-acid active fragment (LKKTETQ) of thymosin beta-4, a small protein the body produces naturally to help regulate the actin cytoskeleton, the internal scaffolding cells use to move and change shape. Because that scaffolding is central to how cells migrate into a wound, thymosin beta-4 and its fragments have been studied for wound-healing applications since the 1990s, well before the peptide’s current popularity in recovery circles.

How It’s Proposed to Work

Researchers describe thymosin beta-4 as helping cells migrate to an injury site, supporting new blood vessel formation, and modulating local inflammation. Most of the supporting research comes from dermal wound, corneal, and cardiac-tissue models rather than tendon or ligament injury, so its use for sports-recovery purposes rests more on extrapolated mechanism than on tissue-specific trial data.

What the Research Shows

Evidence for TB-500 spans laboratory, animal, and early human work, but no completed trial has tested it for tendon, ligament, or muscle recovery specifically.

    Wound-Healing Signaling Pathways

    A January 2025 proteomics study identified thymosin beta-4 fragments in the secretome of human amniotic mesenchymal stromal cells, reinforcing its proposed role in wound-healing signaling.

    Dry-Eye Model: Reduced Inflammation and Improved Tear Measures

    A 2022 study tested recombinant human thymosin beta-4 eye drops in a chemically induced dry-eye model and reported reduced inflammation and improved tear measures. That was a controlled laboratory model rather than a musculoskeletal recovery study, but it is one of the more concrete efficacy signals available for this peptide family.

    Proposed Cardioprotectant Pilot Trial (Congenital Heart Surgery)

    A 2012 paper proposed, rather than completed, a randomized, double-blind pilot trial of thymosin beta-4 as a cardioprotectant during congenital heart surgery. We have not located a published completion report for that trial, and we are not aware of any completed human trial testing TB-500 for tendon, ligament, or muscle recovery specifically.

    TB-500 & BPC-157 in Achilles Tendon Healing (Rat Study)

    A July 2026 rat study of Achilles tendon repair reported that the TB-500 group reached a significantly higher maximum load-to-failure and improved tendon histology versus controls; combining TB-500 with BPC-157 offered no additional benefit over either peptide alone.

Regulatory & Safety Status You Should Know

TB-500’s bulk-substance nomination was also previously placed in the FDA’s Category 2 list, with the agency specifically noting it lacks human exposure data and flagging immunogenicity and impurity concerns tied to peptide aggregation. As of the FDA’s most recent update, content current as of April 22, 2026, that nomination has since been withdrawn by the nominator, which changes its status from Category 2 but does not equal FDA approval.

Regulatory Update — July 2026

At that July 23–24, 2026 meeting, the FDA’s Pharmacy Compounding Advisory Committee voted to recommend adding TB-500 (thymosin beta-4) to the Section 503A bulk drug substances list for pharmacy compounding, alongside BPC-157, KPV and MOTS-c. This is a non-binding recommendation rather than FDA approval — the agency has not issued a final decision, and TB-500 remains unapproved for human use as of this update.
